Our Housing Stabilization Services are designed to support individuals in achieving and maintaining stable housing. We offer the following services:

Housing Consultation

Assisting individuals without waiver or targeted case management in developing a Housing Focused Person-Centered Plan. Our consultants collaborate with clients to identify housing preferences, needs, and goals, ensuring a tailored approach to securing stable housing. This includes helping clients make informed choices about housing transition or sustaining service providers and coordinating with other service providers as needed.

Housing Transition

Providing comprehensive support to individuals seeking to find and move into community-based housing. Services include:

Developing an individualized housing search plan: Assessing the client’s housing needs and preferences to create a strategic plan.

Assistance with housing applications and securing housing: Guiding clients through the application process, including help with paperwork and communication with potential landlords.

Identifying and addressing barriers to housing: Providing solutions for challenges such as poor rental history, criminal backgrounds, or insufficient resources.

Coordinating move-in details: Assisting with logistics like arranging movers, setting up utilities, and ensuring a smooth transition into the new home.

Housing Sustaining Services

Ensuring long-term housing stability by providing ongoing support aimed at helping individuals maintain their housing and prevent eviction. Services include:

  • Developing, updating, and modifying housing support and crisis/safety plans: Regularly reviewing and adjusting plans to address evolving needs.
  • Early identification and intervention for behaviors that may jeopardize housing: Monitoring and addressing issues promptly to prevent escalation.
  • Education on tenant and landlord roles, rights, and responsibilities: Empowering clients with knowledge to foster positive relationships with landlords and neighbors.
  • Assistance with the housing recertification process: Helping clients navigate requirements to maintain housing benefits.
  • Advocacy and linkage to community resources: Connecting clients with services to address issues such as financial hardships or legal challenges that may impact housing stability.

Moving Expenses

Offering financial assistance of up to $3,000 to cover eligible moving-related costs for individuals transitioning from Medicaid-funded institutions or provider-operated living arrangements into their own homes. This non-recurring benefit can be used for expenses such as:

Moving services: Hiring professional movers or renting moving equipment.

Essential furnishings and household items: Purchasing necessary furniture, kitchen appliances, and other household essentials.

Security deposits and application fees: Assisting with upfront costs required to secure housing.

Who Is Eligible?

Eligibility Criteria for Housing Stabilization Services:

  • Age and Enrollment:
    • 18 years or older
    • Enrolled in Medical Assistance (MA)
  • Documented Disability or Disabling Condition:
    • Aged, blind, or disabled as per Title II of the Social Security Act
    • Developmental disability or related condition
    • Mental illness
    • Mental health condition, substance use disorder, or physical injury requiring residential care, now transitioning to the community
    • Learning disability recognized by the Department of Human Services (DHS)
    • Substance use disorder with current enrollment in or on the waiting list for a treatment program
  • Need for Assistance in at Least One Area Due to Disability:
    • Communication
    • Mobility
    • Decision-making
    • Managing challenging behaviors
  • Experiencing Housing Instability:
    • Homelessness
    • Transitioning from or recently transitioned from an institution or licensed setting
    • At risk of homelessness
    • At risk of institutionalization
  • Eligible for Specific Waivers:
    • Brain Injury (BI)
    • Community Access for Disability Inclusion (CADI)
    • Community Alternative Care (CAC)
    • Developmental Disability (DD)
    • Elderly Waiver (EW)
