Refer a Client to Clairview Health Group
Help connect individuals in need with housing support services by referring them to Clairview Health Group. Your referrals allow us to assist more people in securing and maintaining stable housing.
Who Can Be Referred?
Individuals who need Housing Stabilization Services can:
Self-refer by contacting a Housing Stabilization Services provider directly.
Be referred by a provider or case manager (including targeted or waiver case managers).
Information Needed for Referral
To determine eligibility, please provide the following documents:
✅ Assessment (MNChoices, Professional Statement of Need (PSN), or Coordinated Entry) completed within the last 12 months.
✅ Proof of disability (PSN, medical opinion form, or SSI/SSDI recipient status).
✅ Person-Centered Plan (Community Services and Support Plan, Coordinated Care Plan for seniors, or Housing-Focused Person-Centered Plan).
Clairview Health Group will submit the eligibility request to DHS for review. Once approved, DHS will notify both the provider (via MN−ITS mailbox) and the individual (via U.S. mail) of the decision.
